Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP (currently in the process of being updated) is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

Youth Coalition


The BCCHC Youth Coalition is a partnership of youth, adults, and youth focused community organizations who come together regularly to give youth a voice in health planning and to give young people the opportunities and resources to be empowered, and to build the capacity for Positive Youth Development (PYD), youth engagement opportunities, and optimal youth health and wellbeing. The Youth Coalition does this through coalition building, pro-social youth events, and other dynamic and responsive processes.

 

Vision – “The [BCCHC] Youth Coalition provides youth with a platform from which they have created and maintain a community that supports Positive Youth Development and optimal youth health and wellbeing.

 

 

 

 

Our Goals

  1. Create a Community Youth Advisory Board to give youth a voice in Bernalillo County , City of Albuquerque government.
  2. Create a youth resource portal powered by MycommuniytNM.org
  3. Provide pro-social, educational, and professional opportunities.
  4. Create a strong network of Youth focused organizations.
  5. Reduce replication and encourage cooperation among youth practitioners.
