Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP (currently in the process of being updated) is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

Youth Jam is coming soon! They need your help!

Youth Jam is scheduled for April 17th, 2010 from 4-9 PM at Warehouse 508. We have received very few donations.  Our funding does not allow us to buy incentives, prizes, food, and other things that are crucial to getting young people to show up!  We know the economy is tricky right now, but if you are able to make a small donation, or if you have a friend who works for a restaurant or store who can donate a gift certificate or item, that would be awesome.  If you can donate by the end of March, and get us a logo or name you would like displayed, we will put it on our event flyer and talk good about you alllll over the place.

 

 

Additionally, we are creating a book of youth voice and art.  If you work with youth or know any youth, please let them know that we would love for them to submit their writing or art.  If they don’t feel comfortable sharing any identifying information, they can put a fake name on it.  Also, I can scan and return art, so if there is a cool drawing they want included but don’t want to let go of, I can get it back to them!

 

 

Finally, youth jam will have a talent show.  If you know any young people (age 25 or younger) who have a talent they would like to share with their peers, please pass along the attached sign up form or have them email or call me.
