Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP (currently in the process of being updated) is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

It’s time we expand the way we think about health to include how to keep it, not just how to get it back.  

  

The New Mexico Health Equity Working Group has prepared this straight forward, talking points brochure to advance the conversation about re-framing how we think - and act - about our health.  Please share it widely, take it to meetings, post it on your webpage, hand it to your elected representatives, and provoke the conversation.

 

Reframing the conversation about health

 

 All New Mexicans should have the opportunity to make the choices that allow them to live a long, healthy life, regardless of their income, education or ethnic background.
