Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

County Health Rankings Released

The Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute recently released a report ranking counties across the country.  This report ranks counties in New Mexico by many different health factors.  Visit countyhealthrankings.org to see how the counties in New Mexico rank.
