Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

You can now connect with My Community NM, the most comprehensive, regularly updated, bi-lingual, resource directory of health and social services for the uninsured in New Mexico on Facebook.  My Community NM is also the home of the Salud Manual!  You can find resources for health, social service, education, community center, legal, and more at http://www.mycommunitynm.org. These resources are concentrated in Albuquerque, Bernalillo County, and Sandoval County but includes resources state wide.  If your organization isn't listed you can sign up and enter it on the site for free. 

 

Our mission is to provide New Mexicans with a comprehensive, partner-based, up-to-date, bilingual online and print inventory of community resources and information in order to:

  • Improve awareness and accessibility of existing services
  • Build knowledge and advocacy skills
  • Facilitate networking and referrals
  • Document service barriers
