Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP (currently in the process of being updated) is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

Board of Commissioners endorses the Bernalillo County Community Health Council Community Health Improvement Plan

On February 23rd, 2010 the Bernalillo County Board of Commissioners endorsed the BCCHC Community Health Improvement Plan (CHIP).  To see the CHIP, follow this link.  The language in the resolution is below and you can access an electronic copy of the Resolution here

 

A RESOLUTION to request the endorsement of the Bernalillo County Community Health Council, Community Health Improvement Plan for 2010-2015 focused on the health priorities of access to health care and poverty. 

WHEREAS, the New Mexico Legislature enacted the County Maternal and Child Health Act in 1991 and the Bernalillo County Board of County Commissioners appointed membership of the Bernalillo County Maternal and Child Health Council, currently known as the Bernalillo County Community Health Council, in response to provisions of the statute in 1992 and has approved subsequent appointments annually since 1992; and

WHEREAS, the County of Bernalillo with a population of more than 630,000 residents benefits from the ongoing work of the Council which creates a county health profile, engages community to identify and prioritize health issues and needs of Bernalillo County residents; assesses availability of healthcare resources, establishing better access to resources in the County; develops a Community Health Improvement Plan with community members, elected officials, and other community partners; collaborates and partners with programs and individuals throughout the County to decrease duplication of services; and

WHEREAS,   the Bernalillo County Board of County Commissioners supports the ongoing work of the Bernalillo County Community Health Council, acknowledging the importance of the Council’s top priorities of access to health care and the elimination of poverty as a means to create a healthy community and address the root causes of health inequity; and

WHEREAS, the Bernalillo County Board of Commissioners recognizing that through the continued support of county-wide health planning and a focus on strategic partnerships to improve the processes in which the needs of the County residents are addressed and create sustainable solutions that support healthy communities;

NOW THEREFORE, BE IT RESOLVED THAT: the Bernalillo County Board of County Commissioners supports the ongoing work and efforts of the Bernalillo County Community Health Council as an effective and valuable Health Council that benefits the residents of Bernalillo County and endorses the 2010-2015 Community Health Improvement Plan to address access to health care and the elimination of poverty.
