Community Health Profile

The Community Health Profile is a critical first step in identifying community health needs and problems, which then serve as the basis for establishing priorities in the Community Health Improvement Plan (CHIP). The CHIP (currently in the process of being updated) is a five-year, community-wide plan that identifies health priorities and community-wide goals and objectives for addressing those priorities.

Access to Health Care Summit - March 31st, 2010

The Bernalillo County Community Health Council is holding a summit on access to health care for people who are immigrants to the Unites States and do not have documentation.  The community has identified this as a priority so the Council, as a neutral convener will be holding a summit on March 31st, 2010.  The Council will develop a report with a summary of the work and a clear action plan for us and our community partners to continue to address this issue.
